Like many South Korean men, Do Ji-ui must complete his mandatory military service. The skilled plastic surgeon opts to work as a public health doctor, rather than in a traditional military role, expecting a fairly straightforward assignment.
But his hopes are dashed when he is assigned to work on Pyeondong-do, a remote island where few public health doctors want to go. The island is notorious for its tough conditions and harsh environment and is considered one of the least desirable postings in the country.

The first encounter of Ji-ui is with Yook Ha-ri, a devoted nurse at the island’s public health center. Oddly enough, she had left a position at a university hospital before coming to the island.
